Refer a participant to Forward Planning Plus
Send us a participant's details with their consent and we'll take it from there: we contact the participant, complete the service agreement, notify the NDIA of the change, and keep you informed with a named contact throughout. We need participant or nominee consent before we can act on any referral.
How the referral process works
- 1Refer: send us the participant's details and consent using the form below, or by phone.
- 2We contact the participant: we reach out to the participant or their nominee to explain plan management and answer questions.
- 3Service agreement: once they're ready to proceed, we complete the service agreement with them.
- 4NDIA notified: we notify the NDIA of the plan management change and coordinate with any previous plan manager.
- 5Ongoing reporting: you get a named contact and, with the participant's consent, visibility over budgets to support your coordination work.
Registration and the service agreement can usually move quickly once the participant is ready. The NDIA recording the plan management change is a separate step outside our control, so we update you rather than promise a fixed date.

We need participant or nominee consent before we can act on any referral, and budget information is only shared with a support coordinator where the participant has consented to that visibility.
