    What this troubleshooter is

    What it is: 11 investigation categories for an invoice that has already been submitted and has not progressed. Each one sets out what to check, who can correct it, what to have ready, and when to bring the plan manager in.

    Who it is for: support coordinators and providers dealing with a held, queried or returned invoice.

    How it differs from the checker: the Invoice Requirements Checker answers is the required information present? before an invoice is sent. This answers it has stalled — what type of issue should I investigate next?

    What it does not determine: it does not tell you why a specific invoice was rejected. It is not an NDIA tool, it does not confirm pricing or funding, and not every held invoice fits one of these categories. More than one can apply at once.

    Participant or plan details mismatch

    Check this first · NDIA claiming and plan rules

    Symptom: The invoice was queried and the participant's details look like the issue

    What to check

    • The participant's name and NDIS number on the invoice match the plan exactly.
    • The service date falls inside the current plan period, not a previous or future plan.
    • The plan is still active and has not been replaced by a reassessed plan covering that date.
    • The plan management arrangement applied on the service date — a plan can change management type mid-plan.

    Who may need to correct it: The provider, once the correct participant and plan details are confirmed.

    Have ready: The invoice as submitted. The participant's NDIS number and current plan dates.

    When to contact the plan manager: If the details on the invoice look correct and it is still being queried, ask the plan manager what was recorded against the claim.

    Provider identifiers or details

    Provider correction may be needed · Tax invoice requirements (ATO)

    Symptom: The provider's own details on the invoice were questioned

    What to check

    • The legal entity name and ABN on the invoice match the entity holding the service agreement.
    • The ABN is current and correct — a lapsed or mistyped ABN affects tax invoice validity.
    • Bank details belong to the invoicing entity and have not changed without written notice.
    • The document is identifiable as an invoice and carries a unique invoice number.

    Who may need to correct it: The provider — only they can reissue an invoice under the correct entity details.

    Have ready: The invoice. The service agreement or provider onboarding details.

    When to contact the plan manager: If the provider insists the details are correct, ask the plan manager which detail did not reconcile.

    Support item or claim detail

    Plan manager review needed · NDIA claiming and plan rules

    Symptom: The support item or claim detail was flagged

    What to check

    • The support item number and description correspond to the support actually delivered.
    • The item is claimable under the participant's plan and against the category being claimed.
    • The claim type is right where one applies — for example a cancellation or a travel component.
    • The current NDIS Pricing Arrangements and Price Limits for that item, rather than an older edition.

    Who may need to correct it: Usually the provider, after confirming the correct item against the current NDIA pricing arrangements.

    Have ready: The invoice and the support item number used. The plan's funded categories and any stated supports.

    When to contact the plan manager: Where it is unclear whether the item can be claimed against the available funding, ask the plan manager to review before the provider reissues.

    Dates or service period

    Check this first · NDIA claiming and plan rules

    Symptom: The dates on the invoice were questioned

    What to check

    • Service dates are stated, not just an invoice issue date.
    • The service period sits within one plan period rather than spanning two plans.
    • The dates are not in the future.
    • For a period of support, the start and end dates are both shown.

    Who may need to correct it: The provider, by reissuing with the correct service dates.

    Have ready: The invoice. The plan start and end dates. Any service delivery records the provider holds.

    When to contact the plan manager: If the support genuinely spans two plan periods, ask the plan manager how it should be split before it is reissued.

    Quantity, rate or value

    Provider correction may be needed · NDIA claiming and plan rules

    Symptom: The amount, rate or quantity does not reconcile

    What to check

    • Quantity multiplied by unit price equals the line total, and the line totals equal the invoice total.
    • The unit of measure matches the support item — hour, each, day, week.
    • The rate sits within the current price limit for that item, where a limit applies.
    • Any travel, cancellation or non-face-to-face component is itemised rather than absorbed into the rate.

    Who may need to correct it: The provider — arithmetic and rate corrections have to come from the invoice issuer.

    Have ready: The invoice. The service agreement rate, if one is set. The current NDIA price limit for the item.

    When to contact the plan manager: If the rate appears to exceed the current price limit, ask the plan manager to confirm before the provider reissues.

    Duplicate invoice

    Symptom: It may already have been submitted

    What to check

    • The invoice number has not been used before for the same participant.
    • The same service period and amount were not already submitted under a different invoice number.
    • A reissued or corrected invoice is clearly marked as replacing the original, with the original reference.
    • A statement or reminder has not been sent in a format that reads as a new invoice.

    Who may need to correct it: The provider, by withdrawing the duplicate or clearly identifying the replacement.

    Have ready: Both invoice numbers. The service dates and amounts on each.

    When to contact the plan manager: Ask the plan manager which invoice reference is on record before the provider issues anything further.

    Insufficient or unclear description

    Likely information gap · Tax invoice requirements (ATO)

    Symptom: The description was called insufficient or unclear

    What to check

    • Each line describes the support delivered well enough to identify it, not just a generic 'services rendered'.
    • The description is consistent with the support item number claimed.
    • Abbreviations and internal codes are explained or replaced.
    • Where multiple supports are combined, they are separated into distinct lines.

    Who may need to correct it: The provider, by reissuing with clearer line descriptions.

    Have ready: The invoice. The support item claimed.

    When to contact the plan manager: If the provider is unsure what level of detail is expected, the plan manager can describe what they need to see.

    GST or tax invoice issue

    Provider correction may be needed · Tax invoice requirements (ATO)

    Symptom: There is a GST or tax invoice question

    What to check

    • Whether GST applies at all — many NDIS supports to participants are GST-free, and this depends on the support and the provider's circumstances.
    • Where GST is charged, the invoice shows it in a way that meets the ATO's tax invoice requirements.
    • The words 'tax invoice' appear where the provider is registered for GST.
    • GST treatment is consistent across all lines rather than mixed without explanation.

    Who may need to correct it: The provider, with their own accountant or tax adviser. GST treatment is the provider's responsibility.

    Have ready: The invoice. The provider's ABN and GST registration position.

    When to contact the plan manager: The plan manager can say what did not reconcile, but cannot determine a provider's GST obligations.

    Support not aligned to available funding or plan circumstances

    Plan manager review needed · NDIA claiming and plan rules

    Symptom: The support may not be funded or the budget may not cover it

    What to check

    • The support is funded in the participant's current plan.
    • The category being claimed against has remaining funds for that service period.
    • The support is not a stated support being claimed from a flexible budget, or the reverse.
    • The participant's circumstances have not changed in a way that affects the plan — for example hospital admission or a reassessment.

    Who may need to correct it: Nobody can correct this on the invoice alone. It needs a funding review, and possibly a conversation about the plan.

    Have ready: The invoice. The current plan's funded categories. The most recent budget statement.

    When to contact the plan manager: Contact the plan manager. This is a funding question rather than an invoice formatting question.

    Missing required information

    Likely information gap · NDIA claiming and plan rules

    Symptom: Something required is simply not on the invoice

    What to check

    • Run the invoice through the Invoice Requirements Checker to see which fields are absent.
    • Confirm the participant identifiers, service dates, support item, quantity, rate and totals are all present.
    • Confirm the provider's entity name, ABN and invoice number are present.
    • Confirm payment details are stated so payment can actually be made.

    Who may need to correct it: The provider, by reissuing a complete invoice.

    Have ready: The invoice. The checker result showing which fields are absent.

    When to contact the plan manager: If every field looks present and the invoice is still not progressing, ask the plan manager what is outstanding.

    Forward Planning Plus processing preference

    Symptom: Forward Planning Plus asked for something extra

    What to check

    • Whether the request relates to how the invoice reaches us — for example the invoice email address used, or a PDF rather than an image.
    • Whether the participant's name and NDIS number appear on the invoice itself rather than only in the covering email.
    • Whether a corrected invoice was sent as a replacement rather than an amended copy of the original.
    • Whether provider bank details on file match the invoice.

    Who may need to correct it: Usually the provider, but this category is a Forward Planning Plus processing preference, not an NDIA requirement.

    This troubleshooter suggests where to look next when an invoice has stalled, been queried or been returned for correction. It does not determine why any particular invoice was rejected, it is not an NDIA tool, and it is not legal, accounting or tax advice. Not every held or rejected invoice fits one of these categories, and more than one category can apply at once. Where a support's funding is in question, that is a plan and budget matter rather than an invoice formatting matter.
